Start a QuotaGuard Shield BAA review
QuotaGuard can support HIPAA-regulated customers through approved QuotaGuard Shield configurations and a signed Business Associate Agreement.
Before preparing a BAA, we ask customers to complete a short intake process so we can document the intended Shield configuration and confirm whether it is appropriate for HIPAA-regulated use.
What to send us first
Send a short note with the information below. If the use case is a possible fit, we will send the HIPAA / BAA intake form and review the configuration before preparing signature documents.
Do not send PHI, patient data, screenshots containing PHI, private keys, certificates containing private key material, or sensitive secrets through this form or email.
Include this information
- Company legal name
- QuotaGuard account email or account ID
- Whether your organization is a covered entity, business associate, or subcontractor business associate
- Whether ePHI will pass through QuotaGuard Shield
- Whether the use case is inbound Shield, outbound Shield, or both
- Target timing for BAA review
This starts the review process. It does not create BAA coverage or approve a configuration.
